INDEPENDENT PROGRAM DISCLOSURE
Blue Shark Soccer Club LLC is an independent soccer program and is not owned, operated, managed, or administered by Somerset Academy Bay. Blue Shark Soccer Club LLC is solely responsible for the operation of its soccer program, including registration, coaching, program fees, payments, scheduling, communications, and program activities. Use of Somerset Academy Bay facilities or references to the training location do not make Blue Shark Soccer Club LLC part of, or an agent of, Somerset Academy Bay. Questions regarding the Blue Shark program should be directed to Coach Pulido or Blue Shark Soccer Club LLC.

I understand that soccer and athletic activities involve inherent risks, including falls, collisions, contact with players or equipment, sprains, strains, fractures, head injuries, heat-related illness and other injuries that may be serious. I understand that reasonable safety measures cannot eliminate every risk. I voluntarily permit my child's participation and acknowledge these risks. To the fullest extent permitted by applicable law, I agree that any release or limitation of liability in this packet is intended to apply only as legally enforceable and does not waive rights that cannot lawfully be waived. I understand that this document does not replace any rights or obligations established by applicable law.

ANNUAL CONCUSSION & HEAD INJURY INFORMED CONSENT
A concussion is a brain injury and may occur without loss of consciousness. A youth athlete suspected of sustaining a concussion or head injury during practice or competition WILL be immediately removed. The athlete MAY NOT return to practice or competition until Blue Shark receives written medical clearance stating that the athlete no longer exhibits signs, symptoms, or behaviors consistent with a concussion or other head injury, authorized by an appropriate health-care practitioner trained in concussion diagnosis, evaluation, and management.

- Attendance & punctuality: Players should arrive on time and be picked up promptly.
- Conduct: Players and families are expected to show respect, sportsmanship and appropriate behavior.
- Equipment & hydration: Players should arrive with appropriate athletic clothing, soccer footwear as directed, and water.
- Weather: Outdoor sessions may be delayed, modified, moved to a make-up date, or canceled when conditions are unsafe.
- Injury reporting: Parents should disclose relevant restrictions and promptly report injuries or medical limitations.
- Pickup: A child will only be released according to the program's authorized pickup procedures.
8. PHOTO & VIDEO AUTHORIZATION - OPTIONAL

ADDITIONAL GAME, TOURNAMENT & TRANSPORTATION EXPENSES
Monthly training fees and registration fees do not automatically include expenses associated with games, leagues, tournaments, cups, referees, facility or field fees, team entry fees, travel, transportation, parking, lodging, meals, or other competition-related expenses. When Blue Shark Soccer Club participates in an event that requires additional payment, families will be informed in advance of the applicable costs. Unless specifically communicated otherwise in writing, parents/guardians are responsible for their player's transportation to and from games, tournaments, competitions, and other off-site activities, as well as any related travel expenses. Blue Shark Soccer Club does not automatically provide transportation for players.
Participation in the academy does not mean that games, leagues, cups or tournaments are included in the monthly training fee.
Initials required in the printed packetPARENT/SPECTATOR AREA DURING TRAINING & GAMES
During training sessions, games, tournaments, and other Blue Shark Soccer Club activities, parents, guardians, and spectators must remain in the designated parent/spectator area and should not enter the training or playing area unless requested by a coach or in the event of an emergency. To provide players with a positive learning environment and allow coaches to work effectively, parents and spectators should avoid giving tactical instructions, coaching from the sideline, interrupting training, or approaching players during the session. Coaching instructions will be provided by the Blue Shark coaching staff.
Parents are encouraged to support, not coach, from the sideline.
Initials required in the printed packetANNUAL REGISTRATION RENEWAL & PROGRAM FEES
Blue Shark Soccer Club registration is subject to annual renewal. Registration renewal fees, monthly training fees, and other program fees may be reviewed and adjusted annually based on program needs, operating costs, facilities, insurance, equipment, staffing, and other applicable expenses. Any changes to registration or monthly fees will be communicated to families in advance before becoming effective. Continued participation after the effective date of a fee change constitutes acceptance of the updated program fees.

Registration, uniform, administrative, and other one-time fees are generally non-refundable once processed, ordered, or committed by Blue Shark Soccer Club. Monthly training fees are generally non-refundable once the applicable training period has begun. A player's withdrawal, vacation, illness, schedule conflict, school activity, family commitment, or other personal circumstance does not automatically create a refund, credit, prorated fee, or transfer of payment. Blue Shark may consider exceptional circumstances individually when it determines that doing so is reasonable. Nothing in this policy limits any right that cannot legally be waived.

Payments to Blue Shark Soccer Club are made through Zelle to (305) 481-4320. Parents/guardians should include the player's full name in the payment memo whenever possible. A payment is considered received after it has been successfully transmitted and confirmed. Families should retain their payment confirmation for their records.
Initials required in the printed packetPAYMENT DUE DATES & OUTSTANDING BALANCES
Program fees must be paid by the due date communicated to the family. If an account becomes past due, Blue Shark Soccer Club may contact the parent/guardian and may temporarily suspend the player's participation until the outstanding balance is resolved. Blue Shark will make reasonable efforts to communicate an outstanding balance before suspension when circumstances permit. Any late fee or additional charge will only apply if it has been communicated to the family in advance. Blue Shark may establish or modify payment procedures with reasonable advance notice.
Initials required in the printed packetPLAYER ABSENCES, MISSED SESSIONS & PROGRAM CANCELLATIONS
Player absences for illness, vacation, appointments, school events, family activities, transportation issues, or other personal reasons do not automatically entitle the family to a refund, credit, prorated fee, or individual make-up session. If Blue Shark cancels or materially changes a scheduled session because of weather, field conditions, facility availability, safety concerns, or another operational circumstance, Blue Shark may provide a make-up session, schedule adjustment, credit, or another reasonable solution when appropriate and feasible. A particular make-up date or remedy is not guaranteed, and decisions may depend on field availability, scheduling, safety, and circumstances beyond the program's control.
Initials required in the printed packetPARENT COMMUNICATION & COACHING BOUNDARIES
Parents and guardians are encouraged to raise questions or concerns directly with Coach Pulido before or after a session, or through the program's designated communication method, rather than interrupting training or competition. During practices and games, the Blue Shark coaching staff is responsible for player instruction, exercises, tactical decisions, positions, substitutions, and management of the activity. Parents are encouraged to support, not coach, from the sideline. If an urgent health or safety matter arises, parents should notify the coaching staff immediately. Blue Shark welcomes respectful communication and will make reasonable efforts to address concerns at an appropriate time.

Blue Shark Soccer Club may take reasonable corrective action, including temporary suspension or dismissal from the program, when a player's or participant's conduct creates a safety concern, substantially disrupts the program, repeatedly violates program rules, involves serious misconduct, or when required program fees remain unresolved. When circumstances permit, Blue Shark will make reasonable efforts to communicate concerns with the parent/guardian before a suspension or dismissal. Immediate action may be taken when necessary to protect players, staff, families, facilities, or the safe operation of the program. Any financial consequences will be handled consistently with the program's refund and payment policies and applicable law.

Training times, game schedules, fields, and activity locations may be changed when reasonably necessary because of weather, field or facility availability, school or facility events, maintenance, safety concerns, emergencies, league or tournament requirements, or other circumstances beyond Blue Shark's reasonable control. Blue Shark will make reasonable efforts to notify families as soon as practical. Such changes do not automatically create a refund or credit and will be handled under the program's cancellation and make-up policies.

Players and families are responsible for their personal belongings. Blue Shark Soccer Club is not responsible for personal items that are lost, misplaced, stolen, or damaged, except to the extent responsibility cannot legally be excluded. Players are encouraged to bring only necessary equipment and to avoid bringing valuables to training, games, or tournaments. Families should label important equipment when practical.

Parents/guardians are responsible for promptly notifying Blue Shark Soccer Club of material changes to contact information, emergency contacts, authorized pickup persons, relevant medical information, allergies, restrictions, or other information that may affect the player's safety or participation. Blue Shark may reasonably rely on the most recent information provided by the parent/guardian until an update is received.

Participation in Blue Shark Soccer Club does not guarantee a specific position, starting role, amount of playing time, roster placement, or selection for every game, league, cup, or tournament. Coaching decisions will be made by the Blue Shark coaching staff based on factors such as player development, attendance, effort, conduct, safety, team needs, event requirements, and the nature of the competition. Blue Shark's goal is player development while maintaining the coaching staff's ability to make appropriate sporting decisions.

Blue Shark Soccer Club may reasonably update program policies, procedures, schedules, and operational requirements as the program develops or as safety, facility, insurance, league, legal, or administrative needs change. Material changes that affect families will be communicated in advance when reasonably possible. No policy update will eliminate rights that cannot legally be waived.

Additional uniforms may be available for purchase separately. The price of additional uniforms will be communicated to families before an order is placed. Blue Shark Soccer Club may also offer optional official merchandise, including T-shirts, jackets/coats, caps, training apparel, accessories, and other Blue Shark items. Merchandise availability, designs, sizes, and prices may vary and will be announced or published in advance. The purchase of additional uniforms or merchandise is optional unless a specific item is identified as required for participation in a particular team, game, tournament, or activity.

Blue Shark may assign qualified substitute coaches or staff when the regularly assigned coach is unavailable. A staff change alone does not create a right to a refund or credit when the applicable program service continues. Blue Shark may assign or reassign players to the training group, developmental level, position, or competitive environment reasonably considered appropriate based on development, safety, attendance, effort, conduct, technical level, physical readiness, team needs, and available space. Parent/player requests may be considered but do not guarantee a particular group, team, position, coach, starting role, playing time, roster placement, or selection for every game or tournament.

The signer represents that he or she is a natural guardian of the minor child or otherwise has legal authority to execute the applicable consents. This agreement is governed by Florida law. If a provision is determined unenforceable, the remaining provisions are intended to remain effective to the extent permitted by law. Nothing in this packet waives a right that cannot legally be waived.
